About Death Records in Arizona

The Arizona Office of Vital Records maintains and issues certified death, fetal death, and stillbirth certificates. The Arizona Vital Records Office is managed by the Arizona Department of Health Services. Arizona is a closed records state, so only a select few individuals are allowed access to death records. Individuals with access can include the registrant, immediate family members, spouses and legal representatives.

Death certificates can also be obtained through the county health department. In some county health offices, death certificates are available for copying for up to 30 days after the date of death.
